Please fill out the details below for the player attending.
Players must have some experience playing basketball (school/club/college, etc.).
These trials are for our competitive, national league teams.
£6 fee.
Any player wishing to attend must have completed this form prior.
No walk-ins will be accepted.
Trials are closed for spectators.

I confirm that I have declared any medical information that the club may need to consider. I confirm that to the best of my knowledge (player) and my children (parent) is fit and well to play sport.
I am going to accompany my registration by agreeing to the Club Constitution and terms, including paying the season fees/subscription. By submitting this form we (players & parents) agree to adhere to the policies adopted by the Club including the Basketball England Code of Ethics and Conduct.
In the event of an emergency whilst reasonable steps will be taken to contact parents please confirm that you consent to your child receiving emergency first aid treatment
I give permission for photos and videos of me (the player), to be taken and used for club promotion purposes via the club website and social media channels. I understand that photo/video consent lasts for 12 months.
